This video was the fifth of the series produced by System Safety Services and like the others, challenges the viewer to recognize the links that form to cause the accident. The video then demonstrates the links and finally the safety nets that can help prevent a repeat of the accident.

The video is not based on an actual accident but has been made up from experiences and amplified. It demonstrates how stress can cause a person to make a decision that they would not normally make. (See Case Study #5 below)

The video is used in System Safety Services, Human Performance in Maintenance Part 1, (Fixed wing) workshop and could be used in management training.
